Full Version: 401-K converted to a Roth. Two Form 1099-R's?

In 1998 I had a 401-K which I converted to a Roth IRA. Before it could be converted to a Roth IRA it was rolled over into a Regular IRA. This all took place in a matter of a day or two. Should I have been sent with "two" Form 1099-R's? And both have to be reported on my 1040? Thanks for your input!

Yes, you should have received 2 forms 1099-R - one showing the distribution from the 401(k) (code "G" if it was a direct rollover to the IRA or a "1" or "7" if the check was made payable to you and you rolled it) and one showing the taxable distribution from the traditional IRA to the Roth IRA. You'll need to get a Form 8606 to report the conversion. You can get that at the IRS web site: www.irs.ustreas.gov.
