This is a true story! we process a plan for 'SUPER WOMEN of AMERICA' its a 401k, self directed. the plan is top heavy the profit sharing portion is integrated. and it was a prototype. so on the old 5500 forms, we had to indicate this with the following codes: CHIK Truly, a CHIK plan.
