Does anyone have a consent form they have used to document that the plan sponsor agrees to its data being posted on the Web? We know that they have asked for this service, and are paying a premium for it, but are concerned that if there should be some unforseen consequences to the data being on the Web, we want written authorization from the plan sponsors.
