  1. It is hard for me to imagine a court order awarding custody without some language which has the effect of guardianship. I would think it best that you talk to the lawyer that represented the child in the custody proceedings, if there is one (guardian ad litem).

Check the box(es) next to the form type(s) for which you are requesting an additional TCC Note: The IRS encourages transmitters who file for multiple payers to submit one application and use the assigned TCC for all payers. Forms 1097, 1098, 1099, 3921, 3922, 5498 and W-2G (See Publication 1220) Form 1042-S, Foreign Person’s U.S. Source Income Subject to Withholding (See Publication 1187) Form 8027, Employer’s Annual Information Return of Tip Income and Allocated Tips (See Publication 1239) Form 8955-SSA, Annual Registration Statement Identifying Separated Participants with Deferred Vested Benefits (See Publication 4810) This is from the Form 4419 instructions... Block 8 - Only check the box next to the form types for which you are requesting an additional TCC. A separate TCC will be assigned for each box checked in Block 8. Required. (emphasis is mine) So looks like you get different TCC's for each form. We have separate TCC's.

  3. That's a red flag for those of us on this board. Most of us here (I think, at least the ones participating the most) are either owners or consultants of smaller companies that have a hands-on relationship with our clients and know the laws inside and out, or at least know enough to research or ask when unsure. Fidelity is in it for the assets. They may have some fine people on the admin side but they aren't taking the same approach that we are to running a plan. It sounds like someone simply made a mistake - whether it is Fidelity or your employer, I don't know. There used to be a time when contributions were generally limited to lower percentages, and it may be that such a limit is stuck in your payroll system incorrectly. Go back to payroll and ask why you were limited, and if they say because the plan says so, ask to see that section. Don't assume they know what they are doing. Mistakes are made, all the time. Let us know what they say.
