Guest Bud Posted March 7, 2002 Posted March 7, 2002 In the BNA Tax Management Portfolio for fringe benefits, I think there is a sample document in the working pages. RIA checkpoint also has a sample document. They are pretty simple. The Code lays out the requirements and serves as good outline.
