I am interested in obtaining information from our associates regarding their impressions of healthcare benefits, i.e preferences, satisfaction with existing plans, willingness or desire for plan changes, etc.

Would anyone be willing and able to share a template or list of questions which you might have used when conducting such a survey? OR can you direct me to any literature that would assist me in this effort? Thanks!

I think you're doing a wise thing. I can fax you a basic survey. What kind of information are you seeking form employees?

Health Benefits Survey Checklist

Suggestions for giving in-person surveys.

The following checklist offers suggestions for administering in-person surveys:

Make a preliminary announcement of a forthcoming survey.

Prepare a memo to employees indicating when and where the survey will be held.

Schedule small groups to complete the questionnaire.

Allow approximately one hour for each group to complete the questionnaire.

Assign neutral onsite survey monitors (not supervisors or managers).

Distribute blank questionnaires along with envelopes.

Make arrangements for absent or otherwise unavailable employees.

Post survey results to remind employees they're involved.
