Part I of the schedule D......

I am somewhat confused how to correctly complete this section.

Example:

I have a client whose funds are parked w/ Penn Mutual Life Insurace who has funds in Fidelity Equity Income and Putnam Large cap.

So...

PART I

a. name of Mtia, [PSA etc.....: Fidelity Eq Income?

b. Sponsor: Penn Mutual

c. EIN: Penn Mutuals EIN - 000

d. entity code: P

does the above seem correct or should it be:

b: Fidelity

c: Fidelity's ein for the Equity income fund - XXX extension that corresponds with fidelity's classification

Your first scenario is correct - list the actual fund that is included in Penn Mutual's group annuity (Fidelity Equity Income) if that's what it is called. We use some group carriers who have both types of funds, those that use the name of the investment company, such as Fidelity Advisor's Equity Growth fund, as well as funds that do not have the money manager's name, simply the fund name, such as Select Growth.

I think you'll need to ask Penn Mutual.

A schedule D is required when plan assets are invested in one or more common/collective trusts (CCTs), pooled separate accounts (PSAs), master trust investment accounts (MTIAs), or 103-12 Investment Entities (103-12 IEs) at any time during the plan year.

I would contact Penn Mutual (where the money is held) and see what they say. If its one of the above, then they should also be able to tell you what goes in what boxes. If not, then the first list you gave is how I would fill it out. Penn Mutual will need to give you the name of the PSA.
