Does anyone want to weigh-in on whether mouthwash of any brand, over the counter, should be in the dual purpose column or excluded column??

Doesn't the answer depend on whether or not the Claims Administrator decides that it is for "medical care" or for personal (cosmetic) use?

We are the claims administrator, and the majority of claims ajudicators consider the expense for personal use, or general health and therefor should appear on the excluded list - period. The minority, believe that if a participant has a condition and submits a medical practioners note identifying that a condition exists and how long the item should be used, then mouthwash should appear on the dual purpose list.

It seems that the qualifier is the medical practtitioners note, which would put it in the "Allowed" list. However, it is very unlikely that the note would recommend an OTC instead of something like Peridex or Hydrogen Peroxide (32%).

Actually I know of a case where I would put it on the "Allowed" list with a note from the practitioner. Listerine is often recommended by dentists as a treatment for early gum disease. In this case I would cover it.
