Guest MCarey1 Posted May 12, 2004 Posted May 12, 2004 I usually attached Explanation of Premiums to my schedule A's from Insurance Companies that hold life insurance policies. Not sure if this is correct or not, but I have always done it. I indicate "See attached" on the Schedule A line 5a Basis for Premiums. Hope this helps.
Blinky the 3-eyed Fish Posted May 12, 2004 Posted May 12, 2004 I recall the attachment being used before the EFAST system, but not required around the time it came to be. Some insurance companies are slow to adjust and continue to provide signed certifications of the information, although it is not needed. I think MCarey is talking about something different. "What's in the big salad?" "Big lettuce, big carrots, tomatoes like volleyballs."
