Small employers are required to file a Form 5500 if they offer a qualified (retirement plan) to their employees. There are some exceptions to the rules of course. If you only offer welfare benefits (i.e. health coverage), there is no filing requirement unless these benefits are funded through a trust (which is probably not the case).
