Guest JM123 Posted July 13, 2011 Posted July 13, 2011 The DOL website states that late top hat submissions can be made by mail using the most current Form 5500 (with only limited informatio nrequired). I assume it is okay to use the printed form (which is marked "SAMPLE") for this specific purpose. Does anyone know if that's incorrect?

With that understanding, is the question that the DOL was not notified of this in a timely fashion? What are we trying to do? CPC, QPA, QKA, TGPC, ERPA

I should specified that the submission would be under the DFVCP. The other option appears to be to simply file electronically, which does not appear to require any Form 5500 submission for delinquent top hat registrations. I think we would just pay the $750 fee, file a registration statement, and be done with it.
