Form 8955-SSA Individual Statement

What are others using as the individual statement that needs to be furnished to all those reported on the new SSA form? Is there a sample format available that anyone knows of?

This Relius tech update takes the position that for a DC plan a combination of the PPA statement and the distribution form should do it. A DB plan is different.

Relius also offers for people who use their service a draft of a statement one could use.

thanks! That's what I've seen as well, just wondering if anyone else is doing anything different or has a statement format they are using. We don't subscribe to the Pension Library so I cannot access their sample statement
