Guest Dea Posted January 23, 2012 Posted January 23, 2012 My H&W plan terminated their vision benefit during the plan year. On the Form 5500, should I not use code 4E because vision was discontinued or should I still use it for this filing because the vision benefit was offered for part of the plan year? I can't find anything in the instructions that state whether these codes should be used as of the beginning of the plan year or the end of the plan year. Thanks!
JohnCheek Posted January 28, 2012 Posted January 28, 2012 Instructions say "answer all questions with respect to the plan or DFE year", unless otherwise explicitly stated...
