The Employee Benefits Survey conducted by the Department of Labor may provide some useful information.

I saw a Hewitt survey from early 1998 which said that, among the 509 surveyed companies, 17.5% of employees participate in a health FSA.

Average contribution for a health FSA, according to the survey, is $744. For dependent care FSA, avg contribution is $2848.
