Guest julieh Posted February 4, 1999 Posted February 4, 1999 We are currently filing one Form 5500 for our Cafeteria Plan. Our plan includes the following: Dependent Care Reimbursement, Medical Reimbursement, Health Insurance Premiums, and Group Life and Disability Premiums. We have over 100 employees, and some of these plans have over 100 participants. Do we need to file separate 5500's for these plans?
SLuskin Posted February 4, 1999 Posted February 4, 1999 You do not have to file a separate 5500 for your dependent daycare plan. You will need 1 for the Cafeteria Plan as a whole, 1 for the unreimbursed medical FSA if more than 100 participants in it. As for the rest, it depends on how you have them defined. If 1 plan, it will include health, di and life. If 3 plans (3 different plan years, for example) you will need a 5500 for each.
Guest julieh Posted February 6, 1999 Posted February 6, 1999 Thanks Sluskin! Now that I know we have to file separate 5500's; since our cafeteria plan is Plan Number 501, can I just name the others 502, 503, and 504? Would that Plan Number have to be noted anywhere else, or just on the 5500? ------------------ Julie
