Can an officer start to flex health insurance during the middle of the year, if he hadn't completed a form prior to the beginning of the year? The company paid for the officer's health insurance previously but is not going to be paying it anymore.

I don't think so. This was something caused by the employer and not by the provider of the insurance. Therefore, no changes of any type should be permissible.

I'm not sure what the right answer is, but I would be inclined to consider the circumstance you described to be a significant change in his health insurance. But why is this an issue? Your plan should provide that all health premiums paid by employees are paid on a pre-tax basis. Simply amend your plan to make the premium portion automatic.
