Anyone know where I can get a copy of a sample cafeteria plan claim form?

Does the benefit plan have a professional trustee (maybe an inurance company) ? If yes, then I suggest you call him for a stack of claim forms ... that's one of the things that you are paying him for ?

It's just a small plan with maybe 15 or 20 participants. The client administers the plan themselves.

What kind of plan is it ? ....(a Flexible Spending Arrangement, .... ??).

The client might want to consider have an independent TPA handle the service for them. A TPA would provide all necessary forms and documentation and eliminate the liability issues of the employer processing claims in-house.
