This page provides guidance documents relating to Creditable Coverage requirements for employer and union-sponsored plans and model notice documents. New guidance regarding an employer's disclosure obligations to CMS has been posted. Group health plan sponsors must submit the creditable coverage disclosure form electronically via the CMS website at http://www.cms.hhs.gov/apps/ccdisclosure/default.asp.
