Excerpt: Notification of a breach must be made 'without unreasonable delay' but in no event later than within 60 calendar days of learning of the breach. PHR vendors and PHR-related entities must provide notice to each individual who is a citizen or resident of the United States whose unsecured PHR identifiable information was acquired by an unauthorized person as a result of such breach of security and to the FTC. Third-party service providers must provide notice of a breach of security to a senior official at the applicable PHR vendor or PHR-related entity.
