Where to report plan name changes on the 2000 Form 5500 (if at all)?

I was wondering if anyone knew where or even if we are to report plan name changes on the 2000 form? On the old form there was a box that was checked but I can't seem to find anything on the new form except the section to put new plan sponsor information in. Thanks.

I don't believe neither the DOL nor the IRS are concerned with the plan's name or the sponsor's name. I was surprised to find that some of the 2000 software products do not put the sponsor name or plan name on the 5500 schedules (only the EIN/PN). They claim that DOL allows this. You still have to put the plan name and sponsor name on the face of Form 5500 though.

Part 4 of Form 5500 is for changes in the name of the PLAN sponsor.

DOL tracks a plan by the EIN of the sponsor and the three digit plan code. There is no mechanism to notify the DOL of a change in the plan name, however, to be on the safe side, I would attach a statement to the 5500 as to the fact that the plan name changed and showing what the old name was and the new plan name is.

Because there are many plan sponsors who have not followed the plan numbering system (001, 002, 003,...) the DOL is finding that they are getting filings for separate plans of the same plan sponsor with the same three digit plan code. They are looking at the plan name carefully.

To respond to Emiliano... see Regulation 301.6057-2. It appears that the IRS is concerned about plan name changes after all.
