Guest bgvermont Posted October 25, 2001 Posted October 25, 2001 I had a great Cobra notification letter that I used at my last employer, but am having trouble with the disk it was saved on. So, I've been searching the web for hours for a good, up-to-date Cobra Notification Letter, with no luck. Any website suggestions?
KIP KRAUS Posted October 26, 2001 Posted October 26, 2001 I don’t know of any specific site to find a sample COBRA notice, but if you try searching on www.Google.com you may find some notices used by other employers. By the way. For future reference if you want to save any documents you create at work e-mail them to your home computer, and if you don’t have one at home now may be the time to invest in one. Saving information on floppy or CD can be risky, as you’ve found out.
Guest bgvermont Posted October 26, 2001 Posted October 26, 2001 Thanks for the search tip, and I sure wish I had thought of emailing those documents home. Good suggestion.
Guest dfullerton Posted November 19, 2001 Posted November 19, 2001 Let me know if you found one. Perhaps I can assist. Are you looking for The Initial for when an individual is added on to the plan. If so the Department of Labor has this one.
