Does anyone know where I can find a sample WHCRA notice for an spd?

Well, I'm not sure. I found a model annual notice published by the DOL in October of 1999. Since the same publication states that an annual notice may be included in the spd I'm assuming that the model notice they provided can be used for that purpose. I'd like your help unless you think this model is sufficient.

If the model notice was issued by the DOL, then it probably is sufficient and October of 1999 was probably the most recent revision. However, keep in mind that the requirement for notice under the WHCRA is that the participants are to receive notification once every 12 months. Therefore, placing the notice within the confines of the SPD is certainly good but maybe sufficient only if a new SPD is provided to each participant under the medical plan every 12 months.
