mal Posted November 8, 2004 Posted November 8, 2004 Does anyone know a link to checklist of the information which must be contained and disclosed in a retirement packet for a DB plan? We are in the process of reviewing our material and want to be sure it is up to speed. Thanks.
david rigby Posted November 8, 2004 Posted November 8, 2004 Not sure what you mean by "retirement packet". Can you clarify please? I'm a retirement actuary. Nothing about my comments is intended or should be construed as investment, tax, legal or accounting advice. Occasionally, but not all the time, it might be reasonable to interpret my comments as actuarial or consulting advice.
mal Posted November 9, 2004 Author Posted November 9, 2004 The packet of information with the QJSA election material, the explanation of the optional forms of benefits, etc. Given to the person shortly before their retirement.
david rigby Posted November 11, 2004 Posted November 11, 2004 Not aware of a checklist, but you might include - the election form, including J&S explanation, - tax notice if a lump sum is available, - direct deposit authorization, - beneficiary election if applicable, - return envelope, - who/where to call if the check is late, or if address changes, - W-4P (is that the correct form number?), - "have a great retirement" letter, - gold watch. What have I left out? I'm a retirement actuary. Nothing about my comments is intended or should be construed as investment, tax, legal or accounting advice. Occasionally, but not all the time, it might be reasonable to interpret my comments as actuarial or consulting advice.
