When former employee finish the 18 months of COBRA are employers required to send them (employee & spouse) a letter informing them that the 18 months of coverage is over such and such a date?

The new regs require you to send a notice of early termination in a case such as termination for non-payment of premium. You would have the requirement of the HIPAA Certificate of Creditable coverage at the end of the 18th month period.
