Guest fcdeacy Posted April 20, 2005 Posted April 20, 2005 Is there guidance or any requirements out there that specify what form employers can accept participant address changes, banking information changes, name changes, etc????? I am looking at how we accept these changes and if simple verification over the phone is adequate especially in light of all the identity theft that is out there. Thanks, Fred
