Guest susanyb Posted June 8, 2005 Posted June 8, 2005 We are planning on re-doing our Dependent Eligibility Status Forms for dependents on our benefit plans that are over age 19 - and FT students under 25 Does anyone have a sample form that you could share with me? Also any comments about how often you send them out - monthly, twice a year? Thanks
GBurns Posted June 8, 2005 Posted June 8, 2005 Shouldn't this reflect whatever wording is in your PD and SPD rather than be generic or reflecting someone else's situation? George D. oriecat Posted June 8, 2005 Posted June 8, 2005 I think that's why she asked for a 'sample' so she could then modify it to make it fit. I don't have a form, I just send a letter to the employee, explaining the eligibility and requesting proof that their covered dependent is still a full time student and therefore remains eligible.
Guest georgia Posted June 9, 2005 Posted June 9, 2005 we use this to validate 25,000+ overage fulltime students on an annual basis. http://www.communityhealth.state.ga.us/dep...orms/66-082.pdf
