Does anyone have information on a top hat self funded medical plan for the exclusive benefit of highly compensated employees?

likely will be discriminatory, remember self funded must satisfy IRC 105(h)

There are companies who offer fully insured medical reimbursement plans. This may accomplish what you want. Security Financial and Jefferson Pilot come to mind.
