The form instructions do not clearly set forth directions for how to complete such a filing for multiple employers. Have called Cincinnati office of IRS and they have not been able to answer us (and subsequently faxed our questions in writing but still no response). Has anybody done such a filing? We think we will probably try submitting one application with combined data, and one user fee.
