Where can I find a checklist and program for health insurance and HRA Plans?

I hit the enter button too soon. I am looking for a compliance checklist and program: the kinds of things the IRS and DOL will look at.

Someone else can probably provide a link to a published checklist, but we underwent a DOL audit in 2006 and following are the items they requested:

COBRA notices (initial notice and eligibility notice, including historical versions)

Listing of health plan participants who experienced a QE since 1/1/98

Service Provider Agreements

Plan Documents

Summary Plan Descriptions

HIPAA Certificate of Creditable Coverage - procedures for requesting a certificate and sample of certificate

Listing of Terminated employees since 1/1/98

Log of Certificates of Creditable Coverage mailed since 1/1/98

Description of Special Enrollment rules

Lsiting of health plan claims denied due to pre-existing condition

WHCRA notice initially distributed in 1998

Medical plan carriers' Physician Compensation Agreements
