Guest Sieve Posted August 13, 2010 Posted August 13, 2010 Look at Treas. Reg. Section 54.4980F-1, Q&A-11 for what has to be in the Notice. I do not believe there is any model notice because the provisions of an accrual and an amendment to that accrual are too varied for any model notice to be helpful. (I see that you posted the question twice & got the same answer twice . . .)
