The reason is that the only thing that really matters on those forms is the tax ID numbers provided. A plan name change will not result in a change of the tax ID for the plan or the sponsor. What would I put on the actual forms? The name as of the end of the year for which the reporting is being done. But if you used the old one, NOBODY CARES! Lawrence C.
