I recall that when VSP when they changed their formatting, we talked to them about this. They confirmed informally that they are not experience-rated HOWEVER we did not get anything in writing from them.

Pammie57 - I agree with Madison. If you can confirm that there is no indirect compensation received by your vendors (which may push the total received for each one over $5,000) then you are not required to file.
Can you check both the 'trust' and 'general assets' boxes on line 9? A Health and Welfare plan with a trust (VEBA) pays for some expenses out of the trust, but pays others out of general company assets. Would you check both?
