Wording for SAR distributions

Does anyone have any samples on wording for issueing out SARs. Along with retirement plan SARs I will be sending Welfare, LTD and Travel Accident.

Here is a link to the DOL site that has the suggested wording. You can highlight and paste from here then, just modify and fill in information based on the type of plan. Hope it helps.
