Does the plan have to provide a copy of the policy investment statement to participants? If not, do they need to provide a copy if it is requested?

To the best of my knowledge, the IPS is not a document that the plan sponsor is required to make available to plan participants, even if requested by a participant.

To answer your question directly, no, and no (at least not without litigation/subpoena). But why wouldn't the employer want to share a well-constructed IPS with an employee that requests a copy?
