402(f) Notice for Roth 401(k) distributions

Does anyone know if the IRS is planning to release a model 402(f) notice updated for Roth(k) deferrals? I was just starting to draft one, and it's rather a bore. Would be a lot easier to wait if they are going to be doing one!

My understanding is that they have no plans to update in the near future.

If I recall correctly, Sal Tripodi mentioned at a seminar that ASPPA had provided a draft notice to the IRS for their review. Wonder if ASPPA would be willing to release the draft? That way we'd have something to use until 2009 when the IRS updates the notice.
