Guest Ski Fast Posted May 11, 2007 Posted May 11, 2007 Our plan is missing the mandatory distribution amendment. We can not find it in our files. Our former TPA had all of its documents seized by the DOL, so their copy is also not available. Is there a procedure for replacing a lost document?
