Sample Irrevocalbe Election Out of Plan?

Does anyone have a sample irrevocable election out of a plan that they are willing to share?

Austin, The one I use now comes form Corbel so I can't post it. But the one I wrote and used to use went something like this:

I irrevocably elect to not participate in this retirement plan of the employer. I understand that this decision can never be changed and I will never benefit from this plan. I am making this decision of my own free will and I will hold no one responsible for any harm that may com from it.

It's not great. But it's a starting point for you.

We subscribe to Crobel too - where excactly did you get it from? I didn't see it in the plan administrator kit from Relius documents?

We subscribe to the Pension Library and it is in there.

When I used the PC version of documents and printed the entire forms group, it was in there towards the back.

Now I am on the ASP system and get a zip file. The doc ending in "gen.pdf" is the irrevocable election.

I'm ion the same system, but mine is not showing up? I'm selecting pacakge A, and I have gen.pdf, but there is no irrevocable election in there??? I'll submit to Corbel, but I'll ask you too - am I missing something that you know of? Is there some other box to check?

I receive the 'Election Not to Participate' when I choose Package A with the Volume Submitter Document.

Are you processing a Standarized Protoype by chance?
