Loan Leave Suspension - One Year Cumulative?

I have a situation where a participant went out on an approved medical leave for a few months, came back to work and made several payments but is now back on an approved medical leave. My question is this - is the suspension a one time event or cumulative to total not more than a year during the five year loan repayment period?
