I'm told I should include a cover letter with the QDRO when I file it with the court for the judges signature/approval. Any advice on what should be in the cover letter would be greatly appreciated. Thanks in advance!

It might be useful to the court if you identified the divorce or other court document on which the DRO is based. And I'd provide your contact information in the event the judge needs any additional information from you.

Other than that, I'd simply ask the judge to please sign the DRO and return it you.

It might be useful to:

- identify the parties,

- identify the attorney(s), if any,

- identify any relevant dates (if not already in the DRO),

- state whether the plan administrator has reviewed and approved a draft of the DRO.
