If document doesn't specify, what is the measuring year for "break in service"?  Did not  work 500 hours in what 12 month period?

Plan year?

Calendar year?

Employment Anniversary year?

The year the document says to use for Eligibility?

The year the document says to use for vesting?

Despite many years working on pension plans, am I naïve to believe that plan documents that don't specify exactly what is to be considered a break in service (including what the measurement period is) ought not to exist?  How can something that fails to specify what a "break in service" is even be considered to be a plan document?

Always check with your actuary first!

It is very doubtful that it is not specified in the document.  That doesn't mean  that it will be as simple as "the measuring year for break in service is X".  It might refer to something that refers to something else that is dependent on an election in the adoption agreement.

Thank you.  I went back through the document and looked for the pieces.  I was able to follow the thread.  There are different measuring periods for Eligibility and for vesting.
