Plan is a annually valued trustee directed 401k safe harbor match/cross tested profit sharing plan.  The participant gets an annual statement each year after the plan has been valued following the plan year end.

Other than the Summary Annual Report - what other mandatory annual participant notices must the plan sponsor provide to the employee?  

Are there any notices that are only provided upon request of the participant?

Well, there is the list of assets - you know, the one the DOL was required to issue regs on within 6 months of passage of the PPA of 2006...still waiting.

Cross tested plan that isn't top heavy?  I figure it isn't top heavy because you say you are using the safe harbor 401(k) match, which would be unusual for a top heavy plan where the 3% nonelective works so much better from the standpoint of saving money for the employer since the employees have to get the 3% anyway in a top heavy minimum.  Just curious.....
