May 13, 2015 "[Tips include] using the current version of the applicable forms (i.e., Form 5300, 5307, 5310, and 5316), completing and submitting the procedural requirements checklist, not stapling, binding, or folding plan documents, and using 8-1/2 x 11 inch paper... [T]he IRS instructs applicants to be sure that the forms are completed, signed, and dated. Also, applicants should use the employer identification number (EIN) of the plan sponsor, not the associated trust....
