"The last time that the commercial insurance industry tried to popularize these restrictive provider networks and closed drug formularies, was in the 1990s with the advent of closed HMOs. Narrow networks and drug formularies have been rendered politically acceptable as a result of the concessions made in the ACA. As a result, we will start to see these same approaches become far more prevalent in the commercial insurance market, and even Medicare."

[examined] the research and claims that indicate there is a retirement crisis in America and suggests they are understating what Americans will have for retirement income and overstating what they will need.... Syntax Enhancements for Standard Searches

  • Quotation marks can be used to require an exact phrase, such as
    "standard of review"
  • When CAPITALIZED, the words AND, OR and NOT are logic operators, which are especially powerful when multiple words (e.g., synonyms) are grouped in parentheses, such as
(vested OR vesting OR lifetime) AND (retiree OR retirement) AND (health OR healthcare) AND (benefits OR coverage)
