I am trying to help a client file through VCP to bring their plan document up to date.  They missed both the 2010 and 2016 restatement deadlines.   I have a copy of the adoption agreement that was executed in April 2003.  I am trying to track down a copy of the Basic Plan Document that accompanies the Adoption Agreement.   The Adoption Agreement was sponsored by Universal Pensions, Inc. which I believe sold out to Bisys, who then later sold to Ascensus??  The adoption agreement is the Standardized Adoption Agreement.  The bottom of the document indicates # 8016 (8/2001)   Anyone out there that might have access to the Basic Plan Document that accompanies this Adoption Agreement?

2003 should have been a GUST document, but Universal Pensions, Inc. isn't listed with any prototype documents on the IRS' GUST list. They only show as sponsoring three volume submitter documents.

Do you have a copy of the opinion letter for the adoption agreement?  If you do, there is a number on it that indicates which document provider prepared the document.

I have a copy of the Universal Pensions, Inc. Qualified Retirement Plan and Trust Defined Contribution Basic Plan Document 01 with #8000 (8/2001) at the bottom, and it may be what you need.  I maintain a database of basic plan documents for exactly this reason, and occasionally it pays off.  If I'm doing this correctly, a copy will be attached to this post.  I hope it is what you are looking for!

Thank you QG!  I reviewed the BPD you attached vs the Adoption Agreement, and everything matches up.  What are the chances that you have the corresponding IRS opinion letter in your database?  The client adopted the Standardized 401(K) Adoption Agreement.  Thank you so much for your assistance with the Basic Plan Document.

Happy to help!  I do keep IRS opinion letters in my database, but unfortunately I don't have any for Universal Pensions, Inc.  I also don't have an Adoption Agreement for them in my database, so we were just lucky that I came across the Basic Plan Document at some point and saved it.
