Hearing held March 7, 2002, entitled 'Health Quality and Medical Errors.' Excerpt: Not only do avoidable patient errors harm patients, they drive up health costs by requiring expensive medical interventions to correct subsequent problems. For example, adverse drug events and interactions in hospitals are prevalent and costly.
