Gadgetfreak Posted June 19, 2014 Posted June 19, 2014 I attach the policies (loan, deferral, QDRO, Hardship, as applicable) to the back of the SPD. I am not sure it is required but it makes the most sense (to me). ERPA, QPA, QKA
